All newly registered patients will be invited for a health check with a health care assistant. This includes a brief medical history, details of medication and advice about your health including weight, diet, exercise and blood pressure. Please bring a urine sample with you.
In order to register with a GP at Brownhill, we will require the following
completed documents:
Form GMS1
Completed new patient questionnaire (various forms available
according to age of patient) - Child
Under 5 | Child 5+ | Adult
Completed Audit C form for every patient 16 years of age and over.
New baby - please note we cannot register a new baby without the NHS Number which
was allocated following the birth.
As all the above forms require a signature, we regret that we cannot accept them electronically but we have made them available to you here. Please print off and complete and deliver to the practice in person or by post. Please include a telephone number to enable us to contact you if we require any further information. If you do not have access to a printer new patient packs are available from the surgery. Patients are reminded that you will not be registered with a GP at this surgery until your signed completed forms have been received and checked and processed.
